Sample Language

For an unrestricted gift that allows LifeBridge Health-affiliated hospitals and programs to determine how to use the funds based on the most pressing needs, use the following language for your will and/or trust:

 “I, [your name], of [your city, state, and zip] give, devise, and bequeath to [Sinai Hospital of Baltimore, Inc., Northwest Hospital Center, Inc., Carroll Hospital Center Foundation, Levindale Hebrew Geriatric Center and Hospital, Inc., Bridging Life Hospice, Grace Medical Center, Center for Hope and/or The Family Tree] [written amount or percentage of the estate or description of property] for its unrestricted use and purpose.”

If you have named LifeBridge Health-affiliated hospitals and programs in your will or trust, please let us know so we can ensure that your gift is used according to your wishes. Notifying us of your plans will enable us to plan for the use of your future gift. However, if you prefer to remain anonymous, we will keep your name and gift in strict confidence.
